Rare Cause of Pacemaker Lead Interference: Tricuspid Valve Myxoma

چکیده
Introduction: Primary tumors of the heart are a rare finding, and the most common benign tumors are cardiac myxomas, accounting for up to 80% of cases. They are of endocardial origin therefore the prevalence of myxomas in the atrial septum is highest. Among myxomas left atrial ones are the most common (75%), followed by right atrial ones (18%). Case Presentation: We present a case of a cardiac myxoma of the tricuspid valve with a very unusual clinical presentation of ventricular pacemaker leads interference. A 71-year-old male with history of chronic atrial fibrillation, acute coronary artery syndrome, heart failure with ICD-CRT(Implantable cardioverter Defibrillator-Cardiac Resynchronization Therapy) implantation and mitral regurgitation. A transthoracic echocardiogram showed a right atrial filamentous mass originating from the posterior leaflet of the tricuspid valve and causing inappropriate sensing of the ventricular pacemaker leads. He underwent resection of the mass which on pathology was revealed to be a cardiac myxoma. Conclusions: Cardiac masses are relatively rare findings and the clinical presentation is characterized by nonspecific signs and symptoms, such as embolism, dyspnea, pulmonary edema, fever, fatigue and weight loss, which lead to a wide differential diagnosis. It is crucial for clinicians to consider cardiac myxomas among the possible diagnoses. This case report described an unusual clinical presentation of cardiac myxomas.

Giant myxoma causing right ventricular outflow tract obstruction.
Atrial cardiac myxoma is the most common benign cardiac tumour. Atrial myxoma most commonly arises from the left atrium and, less frequently, from the right atrium or both ventricles. Cardiac myxoma arising from the tricuspid valve is rare. These tumours can present with right heart failure as a result of right ventricular outflow tract obstruction. A high index of suspicion and appropriate inv...
متن کاملCardiac myxoma: an unusual cause of sudden death in childhood.
Primary tumors of the heart and pericardium are rare in autopsy series. Considering all age groups, the most common cardiac tumor is the myxoma. They may arise in any of the four chambers or, rarely, on the heart valves. About 90% are located in the atria, with the left-to-right ratio of 4:1. The majority of patients are usually in the age group of 30-60 years. Female predominance has been repo...

Pacemaker infection has multiple risk factors. Its presentation is most often similar to infected endocarditis and the diagnosis is made through studying blood cultures. Transesophageal echocardiography can confirm the diagnosis. The most common microorganisms are staphylococcus speciesis. As a matter of fact, complete pacemaker removal appears to be the only definite treatment. We presented a ...

INTRODUCTION Cardiac myxoma is a benign neoplasm representing the most common primary cardiac tumor in adults, however it is unusual in neonates. It is represented by an endocardial mass that occupies the cardiac chamber. Although the majority of myxomas are attached to the fossa ovalis of the interatrial septum, they also attach to the walls of the cardiac chambers and to valve leaflets surfac...
متن کاملA probable right atrial myxoma prolapsing through the tricuspid valve into the right ventricle: a case report
INTRODUCTION Myxoma is the most common primary cardiac tumour. This case report illustrates the case of a probable right atrial myxoma prolapsing through the tricuspid valve into the right ventricle, and the fatal outcome if such a mass is not promptly detected and excised. CASE PRESENTATION A 51-year old man presented with a 1-year history of recurrent pedal and abdominal swelling, and 6-mon...
